First Visit Guide
When you come for your appointments, please bring the following:
- Driver’s license or a valid ID
- Insurance information (please try to supply this information to our office at least 2 business days prior to your appointment to facilitate verification of benefits, in-network status and minimize wait time for your appointment)
- Please fill out the Patient Registration Forms prior to your appointment to minimize your wait in the office
- Referral letter (if required)
- Reports, X-rays, MRIs, CT scans etc. and any other relevant information
- List of medications (if any)
